Insurance Plans

We currently participate in the following managed care plans:

Humana/Choice Care, UnitedHealthcare, Definity Health, UMR, Wausau, FISERV, Midwest Security, Golden RUle, Oxford Health, Anthem Blue Cross/BlueShield,  Arise Health Plan, WEA, Health EOS Network, Aetna, (Medicare, Medicare Advantage Programs (AARP, Evercare,Humana Gold, Smart Value, Anthem Medicare, Advantra, Pyramid, Secure Horizons, Evercare) Wisconsin Medicaid (BadgerCare, Title 19), Medicaid UHC Community Care, HIRSP as well as most other major medical insurance companies. If your insurance plan is not listed, please contact us as we may have recently joined your insurance plan’s network. 

It is the patient's responsibility to contact the insurance company to verify your employer's plan participation with our Clinic.  

Our lab specimens are sent to Bellin Lab for processing.  Please check with your insurance plan to verify that Bellin Lab is in your network.  Call us in advance if we need to make arrangements to have your specimens sent to another lab facility. 

We ask that you bring your insurance and an ID card with you on each visit in order to expedite billing. We are happy to file insurance claims for you. We do ask that deductibles, co-insurance (your portion) be paid prior to services. Payment is expected at the time that services are rendered.   Please refer to the Office Policy tab for more information about our financial policies.  Please feel free to discuss financial matters with us at any time. These issues are best directed to our Billing Office staff or office manager.

We are on staff at Bellin Hospital and St. Vincent Hospital for obstetrics, surgeries and hospitalizations.

Independent Clinic

We are a privately owned and operated Medical Clinic.  We are members of Physicians Partners Limited, which is a co-op of other independent medical clinics in the Green Bay Area and Bellin Hospital.  Our membership in PPL allows us to join forces with Bellin and other medical clinics to work together in continuing medical education, service solutions, insurance contracting and benefits for our employees. Belllin provides us the opportunity to provide OB-GYN services in Brillion, Manitowc and Seymour. This co-operative venture prevents expensive duplication of services and provides our patients with easy access to services.
