Kathlene Kusiv, D.O., and Nicky Plementosh, M.D. are on staff at Bellin and St. Vincent Hospitals in Green Bay. 

We see patients Monday thru Friday at our office at 1537 Park Place, Ashwaubenon.

Dr. Kusiv also sees patients on the last Tuesday afternoon of the month at SEYMOUR BELLIN HEALTH  405 Commercial St., Seymour WI  54165.

Dr. Plementosh sees patients every other Friday morning at BRILLION BELLIN HEALTH  964 W Ryan St Suite B., Brillion, WI  54110.
